Vasos sanguíneos no disco óptico de diabetes podem evoluir para uma cegueira.

A injeção de corticosteroide triamcinolone dentro do olho pode retardar a evolução da retinopatia diabética, lesão na retina causada por complicações da diabetes mellitus – podendo resultar em cegueira. A progressão da retinopatia diabética acontece quando se formam vasos sanguíneos no disco óptico ou outro componente da retina.

Apesar dos avanços nos tratamentos para a diabetes, cerca de 70 mil americanos têm o problema. Mais de 60 mil casos novos são contabilizados anualmente. O controle dos níveis de glicose no sangue pode ajudar a prevenir a retinopatia e tratamentos a laser podem reduzir o risco de perda da visão, mas outros tratamentos são desejáveis.

O estudo conduzido por Neil Bressler, da Johns Hopkins University School of Medicine em parceria com o Diabetic Retinopathy Clinical Research Network, envolveu 693 participantes e 840 olhos com edema macular (vazamento de fluido em parte da retina prejudicada pela retinopatia). Alguns receberam o tratamento a laser que destruía os vasos sanguíneos. Em outros foram injetados de 1 a 4 miligramas de triamcinolone acetonide a cada quatro meses. Após dois anos, a retinopatia progrediu em 31% dos 330 olhos tratados a laser, 29% dos 256 olhos tratados com doses de 1 miligrama de triamcinolone e 21% dos 254 olhos tratados com 4 miligramas.

Ao que tudo indica, corticosteróides interferem na criação de novos vasos sanguíneos. Por outro lado, esteróides são normalmente associados a outros tipos de problema de visão, como glaucoma e catarata.
